Hospitals and Medical Facilities

If you plan to intern with a hospital or medical facility you may be required to upload various immunizations, background check results, and drug screen results to the Student Document Obligations page linked to your internship application. After your department internship coordinator approves your internship application, you will be notified (via email) that these student obligations apply to you.

You may begin to collect your required student obligations at any time by uploading them to your Student Document Obligations page.

*IHC has granted an exemption to these student obligations if you are a current employee or volunteer. If you are a current employee or volunteer, please complete this form, including a copy of your badge. Then, upload it to your Student Document Obligations page, and select ‘Current Employee or Volunteer’ as the document type.

MountainView Hospital, Timpanogos Regional Medical Center, St. Mark’s Hospital, Lone Peak Hospital, Mountain View Hospital, Eastern Idaho Regional Medical Center and other independent hospitals are included under a single internship agreement and require the same student obligations (see below). If you are unsure if your internship provider is included in this category, please contact our office for a clarification.
